2023 Chicago’s Magnificent Nomination Form

The Greater Illinois Chapter – Chicago Office of the Cystic Fibrosis Foundation (CFF) and Chicago’s Magnificent will be honoring Chicago’s best and brightest as they compete in a 10-week fundraising competition starting fall 2023. Nominated competitors show exemplary leadership in their lives and are active in their communities. These leaders are not only making a positive local impact, they are also committed to supporting the Foundation’s mission -to find a cure for all people living with cystic fibrosis (CF).

Those who participate in this 10-week fundraising competition are given the opportunity to display their community involvement and highlight their achievements, while raising funds to help provide all people with cystic fibrosis the opportunity to lead full, productive lives.

Chicago’s Magnificent competition begins with an honoree kick-off reception where the 2023 competitors and their respective nominators will be introduced. Competitors will also receive a fundraising guide, a CF ambassador, and a fundraising mentor. Throughout the campaign, one-on-one meetings and networking events will take place. The campaign culminates with a Final Celebration event where competitors are recognized (date and event details to be determined)!

I understand that after submitting my nomination, the nominee will be contacted by the CF Foundation. My nominee will be told I am their nominator and will be invited to submit a formal application to be an honoree for Chicago's Magnificent. The nominee's application includes further contact information and answers to a few short form questions. I also understand that selected honorees will be asked to raise funds for the CF Foundation as part of Chicago's Magnificent program requirements.
